Joining My Journey: Honoring the Person and Their Story

A favorite song comes on and someone begins to sing along. The smell of apple pie brings back the kitchen where holiday meals were prepared for years. A cup of tea, served the way a person has always liked it, makes an unfamiliar afternoon feel a little more comfortable.

These details may seem small, but they tell us something important: memory loss does not erase the person. Every resident who comes to The Cedars brings a lifetime of experiences, relationships, routines and preferences. Knowing that story helps our Memory Care team provide support that feels personal instead of simply following the same routine for everyone.

The Cedars Joining My Journey

What is Joining My Journey

Joining My Journey is The Cedars’ approach to learning who each Memory Care resident is, both past and present. Families help us understand the person’s career, family life, hobbies, favorite foods and music, daily routines, and the things that make them feel safe and at ease.

That knowledge becomes part of everyday care. A resident who spent years gardening may enjoy arranging flowers or getting their hands in the soil. Someone who sang in church may respond to familiar hymns. A person who always started the afternoon with tea may find reassurance in continuing that routine.

The goal is not to recreate every detail of the past. It is to understand what matters to the person so we can create more opportunities for comfort, enjoyment and connection today.

Meeting residents where they are

Memory loss can change how someone experiences time, place and conversation. Correcting every forgotten date or mixed-up detail can create frustration or anxiety without helping the person feel more secure.

The Joining My Journey approach focuses on the emotion and meaning behind the moment. If a resident is looking for someone from long ago, our team can listen, offer reassurance and gently guide the conversation toward something comforting. The question is not always, “How do we bring this person back to our reality?” Sometimes the better question is, “How can we understand what they need right now?”

This allows residents to live in the present without being repeatedly reminded of what they cannot remember. It also gives staff a more personal way to anticipate needs and recognize what may be causing discomfort, restlessness or withdrawal.

Familiar experiences can still create joy

The five senses can offer powerful points of connection. A familiar melody, the texture of yarn, the scent of warm pie or the taste of a favorite meal may bring recognition or simply create a pleasant feeling in the moment.

That is why activities at The Cedars are connected to residents’ interests whenever possible. Musical entertainment, conversation, familiar household tasks and opportunities to reminisce can all help residents participate in ways that feel natural to them. The response may be a story, a smile, a few words of a song or a relaxed expression. Each one matters.

Families help us know the whole person

Families are an important part of Joining My Journey because they know the stories and details that may not appear in a medical record. What did your loved one do for work? What music filled the house? Did they prefer quiet mornings or a busy kitchen? What made them laugh? How did they like their coffee? Which routines helped them settle in at the end of the day?

Those details give our team more ways to connect. They help us see the parent, spouse, neighbor, teacher, farmer, nurse, business owner, musician or friend behind the diagnosis.

Your loved one still has their own road to travel. At The Cedars, Joining My Journey helps us walk beside them with patience, respect and a better understanding of who they are.
